Our Mission

The prestige of being cared for by someone who knows you.

We deliver high-quality, compassionate care built around each client's needs — never a standard package. Our bilingual and bicultural caregivers bring the highest standard of service, always with an eye toward independence, dignity, and trust.

How It Works

Six steps to program eligibility

1

Assessment

Contact Pennsylvania's Independent Enrollment Broker (IEB) or your local County Assistance Office (CAO) to begin. A Functional Eligibility Determination confirms a nursing-home level of care is needed.

2

Medicaid certification

A doctor completes a Physician Certification Form (MA-51) confirming the need for long-term services.

3

Financial review

The CAO reviews income and assets against program limits. Special rules, like MAWD, may allow those over the income limit to still qualify.

4

Service planning

An assessment is completed to develop an Individualized Service Plan (ISP) that determines the services you need.

5

Waiver enrollment

You're enrolled in the applicable waiver program — for example, Community HealthChoices.
