How It Works
Contact Pennsylvania's Independent Enrollment Broker (IEB) or your local County Assistance Office (CAO) to begin. A Functional Eligibility Determination confirms a nursing-home level of care is needed.
A doctor completes a Physician Certification Form (MA-51) confirming the need for long-term services.
The CAO reviews income and assets against program limits. Special rules, like MAWD, may allow those over the income limit to still qualify.
An assessment is completed to develop an Individualized Service Plan (ISP) that determines the services you need.
You're enrolled in the applicable waiver program — for example, Community HealthChoices.
